Push Ups Again

Mixing things up is what I am doing this week. I want to do some push up exercises. It has been a while since I have been able to do them, and I just gave it a try and see if I am able to crank out 20 very controlled, and very slow push ups. This is a good start. Considering that I have not been able to do any for the last 5 months. It is all about working toward what you want to do. Now I can add them to my fitness workouts. Strengthening my shoulder has worked, and now I can do push ups again.

New custom soft lenses such as Hydrocone (Medlens Innovations, Inc) with steep aspheric optical curves can provide stability, centration, and comfort on eyes which have apical asymmetry (which makes fitting symmetrical gas permeable lenses or piggyback systems difficult). These lenses can be fitted to very steep corneas; they also come in a large range of powers. Base curve, optical zone and diameter can be adjusted to improve the fit and vision. Patients with very sensitive eyes or who have a low comfort threshold are also good candidates for custom aspheric soft lenses. Understanding the back surface geometry of a soft lens is as important to fitting these lenses as it is with a gas permeable lens. Custom aspheric soft lenses can be ordered with prolate or oblate aspheric curves.
When fitting custom soft lenses, there are two potential drawbacks. The first is these lenses are often quite thick. It is common to observe small lacy neovasculariztion at the limbus moving into the cornea with low Dk lenses. Since custom soft torics and aspherics are often very thick, the oxygen getting to the cornea is significantly decreased. Neovascularization is particularly important to watch in patients who have had or are expecting PKP. The second disadvantage is fluctuation of vision. It is common for the vision to fluctuate during the day with this type of lens. Vision could vary two or three lines on an acuity chart. This may be related to lens hydration.
The ability to correct higher order aberrations with adaptive optics using soft contact lenses is new technology that is currently being studied. In the near future soft lenses should be available with adaptive optics which can provide optical correction for irregular corneas. These lenses do not rely on rigid surfaces and the post-lens tear film for correction. As higher order aberration refractions become more clinically available, the ability to correct difficult irregular corneas may make soft contact lenses the optical lens of choice as well as the comfort lens of choice.
